About us
High-quality access to care, enabled through technology.
HealthSnap builds the AI-enabled platform for clinical care delivery, the system that lets healthcare organizations care for chronic populations continuously rather than episodically. Founded in 2019 and based in Miami, we work with 200+ health systems and provider organizations across 33 states and have monitored more than 120,000 patients.
Why we exist
Move chronic care from reactive and clinic-based to continuous and platform-run, and prove it in the literature.
We believe in the power of prevention over treatment. After our co-founders experienced how modern healthcare failed their loved ones, they knew there had to be a better way. We believe that every patient deserves to receive the right care, at the right time, and in the right location, regardless of their condition or status.

Needs headshots and biosThe Chief Scientific Officer and Chief AI Officer are named deliberately — a research function and an AI function with owners is the argument this page is making.
